Which sizes do I need for an iOS app icon?

Since Xcode 14 one 1024 × 1024 px image is enough: Xcode makes the smaller sizes itself. “Download for Xcode” gives you that image in an AppIcon.appiconset folder, ready to drop into your project.

Why does Apple reject my icon?

The App Store icon must be square, 1024 × 1024 px and without see-through parts. Don't round the corners yourself: iOS does that. Icons from this maker are always square and solid.
